Labeling the "# ID 1" text box... that I labeled previously as "1", and renumbering it "0" corrected the duplicate / missing column header issue.

You have "ARI Smart Content: DataTable Pack" extension. "ARI Smart Content" and "ARI Data Tables" are two different extensions and this topic is about "ARI Data Tables" extension.
