It is not an issue. You try to load non-existing file. It is not necessary to enter "public_html" into a browser, your site is loaded site from root directory and as we correctly understand it is "public_html" directory.

Could you send a screenshot from a file manager (usually hosting panel contains it) where we can see the file and path to the file?

attached from my FTP interface.. again , im sure its something easy I am missing..

the site is tylersearch.net but when I "get info" on the UB.csv it points to the bob.machighway server

"public_html" is not a root folder for your Joomla! site. It doesn't contains Joomla! folders and files. Enter a path to CSV file on your server. If you don't know it, you can ask hosting support or we can try to help if you provide a temporary access to your Joomla! backend by email and a link to a page where you try to use the module.
