I've read nearly every post and repeated steps multiple times, please tell me where I'm screwing up.

Hello,

It seems this is a problem with selected module position. Open "Extensions -> Template Manager" page on Joomla! backend, click by "Options" button in toolbar and set "Templates -> Preview Module Positions" parameter to "Enabled". After this use this link to see available module positions on the page.

Check also "Menu Assignment" parameters section in module settings.

I had the "Menu Assignment" set to 'No Pages'.
As usual the problem was between the keyboard and the chair.
