Is it possible to add text fields before the quiz starts such as "Enter Your Name", "Enter Your Company", etc? And this info show up with their quiz result?

You can add custom fields before passing quiz and use values of these fields in 'Text Templates'. It requires editing quiz.html.php template file.

Hi, thanks. Wanted to make sure before I started editing the quiz.html.php file. In that file I can put text input form fields for "Your Name" and "Your Company" that will show up at the start of the quiz and those input values will show up in the quiz results in the admin area?

Actually I see in the Results the "User" field column, if we could just utilize that that would be all we would need.

What we're setting up is one quiz that different company's employees will be taking. We'll send different companies a hidden link to the quiz, and we're trying to do this without them having to login but be able to somehow know which company is taking the quiz and be able to separate their results in the admin.

Is there a way to populate the User column with "Company A", "Company B", etc, instead of that just showing "Guest" without having the person login? Would I still edit the quiz.html.php for this?
