hi

i have installed and activated(enabled) the system plugins, when i add the code nothing displays in the frontend. the module im calling is enabled, i have no editor enabled.

Now the link is shown, but the module can't be loaded because it uses outdated "document.write" javascript code. You can load the module into an article and then load the article into the lightbox to avoid the problem with form module. This approach is described in Troobleshooting section. See section #4.
