If you use "ARI Sexy Lightbox" module then select "Images gallery" in "Choose type" parameter, select "Custom text" in "Layout type" and use the following code in "Custom text -> Template" parameter (for example if custom thumbnail is <joomla_directory>/images/gallery.jpg):

PS: We can't login with the provided credentials. We can setup the extension if you provide correct credentials by email and specify on what page you want to show gallery and path to folder with images and to custom thumbnail.
