You can read here how to implement custom theme. If it doesn't help, could you provide a link to a page where quiz with custom theme is used and send you custom theme by email?

I mean, it worked before the changes with the template folder has been introduced. An active Quiz can be found here but as you see in the screenshot (post before) I had to go back to the standard theme. The "old" theme CSS I created can be found here.

To Kupol: about folder with custom language files, these files can be placed into wp-content/languages/plugins folder.
