Do you host your site on wordpress.com and the problem occurs on this hosting? If yes, contact hosting support, probably they have some limitation so the code doesn't work properly.

It is not clear what's the problem with FTP access on Pantheon hosting. It supports this according to their official site.

BTW, we can do nothing with granted permissions for WordPress user. We need have ability to re-install the plugin.

Okay, I'll send it to you today at info@ // Pantheon does not support SSH protocol so I believe you'd need my login credentials which I am loath to give out. Would it be possible to examine the test environment's code via the Wordpress login first to determine the error's source?

We can try to investigate only with credentials for WordPress, but with granted permissions to re-install the plugin.

I've given you WP credentials for both DEV and Test environments. This should allow you to re-install and install the plugin. Let me know if you need anything else, and I hope we can get this plugin up and running ASAP.
